Registering A New Student
To register a new student, parents can create a Clevr account by visiting clevrcloud.com/clevr and clicking “Request Access Here.”

After creating an account, parents will have the opportunity to complete the virtual enrollment form and upload any of the following materials:
- Birth Certificate
- Last Report Card If Applicable
- Immunization Record*
- IEP For Students With Special Needs
- any court papers, should there be any
- Proof of Residency, such as:
- Pay stub
- Income tax form
- Deed or lease to house or apartment
- Utility or other bills sent to the student’s home address
- Membership documents – such as library cards – based upon residency
- Voter registration document
- Official driver’s license, learner’s permit or non-driver I.D.
- State- or other government-issued ID
*New York State Law Requires Parents to have a copy of child’s immunization record and proof of residency.
Important Student Transfer Notice
Attention to any students who plan to transfer to Salmon River! The Salmon River Central School District’s transfer policy deadline is June 1.
This policy applies to all students who wish to transfer into the Salmon River Central School District (including the St. Regis Mohawk School) who are changing school districts but not residency.
Enrollment packets are available in the Office of Instruction, located in the Elementary of the Salmon River Central campus or at the St. Regis Mohawk School. The student’s birth certificate and immunization record are required at the time of enrollment.
Please be aware that the following New York state requirements are necessary for entry: Grades 6-10, a Tdap vaccine, two Varicella, and four Polio vaccines; Grades 7-11, Meningococcal vaccine; and Grade 12, two Meningococcal vaccines. Please check with your health care provider for New York State requirements of your child’s vaccination schedule.

The following forms are to be used for all new enrollments with the exception of the Medicaid Consent. The Medicaid Consent should only be filled out for students that receive Special Education services, or in the case where a parent is unsure if this applies to their child.

SRMT Health Services Release
This form is optional and can be used to allow the school district to access immunization records and/or other health records from SRMT Health Services.
